What are the requirements?

Proof of immunity must include:

  • Three or more doses of diphtheria, tetanus vaccine. One dose MUST be a Tdap. The most recent vaccine must have been administered within 10 years of the students enrollment date.
  • Two measles immunizations, both after one year of age and after December 31, 1968
  • Two mumps immunizations after one year of age
  • Two rubella immunizations after one year of age
  • We strongly recommend receiving 2 MMR vaccinations
  • meningococcalconjugate vaccine for students age 21 or younger. A 2nd vaccine MUST be given if the 1st vaccine was given before age 16. PLEASE NOTE: Meningitis B does not meet this requirement.

All immunization dates must be signed/certified by a physician or public health official. A signed copy of the high school immunization records is also accepted.

How long do I have to submit my immunizations?

Documentation of immunity must be complete and on file in the Student Health Services on the 10th class day of the entering semester.

What happens if I miss the deadline?

If no proof of immunization or certificate/statement of exemption is provided by the 10th class day of the term, the student will not be permitted to register for a subsequent term, add or drop classes for the current term or receive transcripts, and a non-compliance fee will be assessed.

You require a copy of my immunization record. Where do I get it?

Contact your doctor, the County Health Department, or obtain a copy of the 9th grade physical from your high school.

I'm "old." Am I still required to turn in my immunization records?

Call Patient Support Services at (309) 438-7559 for questions about which immunization requirements apply.

I'm in the military. Do I still need to send in my records?

Yes.

Will my military records fulfill the immunization requirements?

Probably not. You will also need proof of your first MMR which is usually given at about 15 months of age.

Do I need to have a physical?

No. (Athletes may have special requirements. Check with a coach or trainer.)

Do I need to have a TB test?

TB tests are only required for international students and must be done at ISU once classes start.

Are the meningitis and Hepatitis B vaccines required by ISU?

The Hepatitis vaccine is recommended by the Center for Disease Control (CDC), but not required at ISU. One meningococcal conjugate vaccine for students age 21 or younger is required. A 2nd vaccine MUST be given if the 1st vaccine was given before age 16. PLEASE NOTE: Menomune and Meningitis B do not meet this requirement.

My doctor does not have the meningitis immunization. Can I get it at ISU?

What is the cost of an immunization?

Prices are subject to change. Please call the Patient Support office at (309) 438-8793 for the most up-to-date prices.

What if I'm taking online classes, and do not live on campus?

Students who do not live on campus, and are not taking on campus classes do not need to turn in immunization records. If you have specific questions about your situation, please feel free to contact Patient Support Services at (309) 438-7559.
