Your Student Health Services medical record is strictly confidential.
Your medical record contains your personal health history and assures continuity of care. This record is kept confidential and is not released to anyone without your written consent, except in compliance with state and federal laws (i.e., subpoena, court order, health department reporting requirements, Notice of Privacy Practices etc.)
Student Health Services maintains all patient medical records for at least six years after your last visit.
Medical Record Copies
Do you need copies of your medical records?
Copies of your medical record on file with Student Health Services are available with signed consent for release (Please Note: The form can be filled in online, once downloaded, but for submission you will need to print the form and sign it manually). Copies are available for pickup after 1:00 PM on the next business day, Monday through Friday. Picture identification is required to pick up your copies. Copies can also be mailed to the address you specify on the signed consent for release. You may submit the consent for release form in person, at the Student Services Building, room 203. You may mail the form to Illinois State University, Student Health Services, Campus Box 2540, Normal, IL 61790, attn Medical Records.
You can also electronically submit your completed consent for release. Other non-immunization records may also be submitted to us using this link. With the exception of immunizations, any outside medical records submitted to ISU Student Health are considered the property of the original medical provider and cannot be re-released by ISU Student Health, even to the patient. To obtain a copy of such records, the patient will need to contact the originating provider.
